I have been working with immunological, clinical and therapeutic aspects of American tegumentary leishmaniasis in the endemic area of Corte de Pedra located at southeast of Bahia, Brazil, for the last 28 years. I began my research activities during my fellowship in Immunodermatology in INSERM U209 at Lyon, France, where I had the opportunity to study the immunopathogenesis of autoimmune bullous diseases. After coming back to Brazil, I began to study clinical and immunological aspects of leprosy and tegumentary leishmaniasis including specific aspects of early disease, or severe forms like mucosal and disseminated leishmaniasis. During this period our group from the Immunology Service at the Federal University of Bahia have investigated several aspects of the host immunity in tegumentary leishmaniasis, contributing to the understanding that a major factor associated to the tissue damage and disease is the exaggerated immune inflammatory response. As a consequence, we have been proposing newer therapeutic approaches like GM-CSF and pentoxyfilline associated with chemotherapy in the successful treatment of cutaneous and mucosal leishmaniasis. In the last few years I have been involved with the design and implementation of clinical trials in different forms of American tegumentary leishmaniasis, with a special interest in immunotherapy associated with specific treatment and also in the evaluation of new anti-Leishmania drugs.
The burden of tegumentary leishmaniasis in the endemic region of Corte de Pedra is associated with several social, economic and psychological consequences constituting an important and urgent public health challenge. I am enthusiastic and looking forward to participating in ECLIPSE, which is designed to explore those aspects in order to ensure a positive impact in the rural communities affected by this neglected disease. The Federal University of Bahia has a strong commitment to social transformation by educational activities and opportunities. ECLIPSE aligns well with our University’s strategy to increase international collaboration and promoting sustainable development in public health with special attention to endemic diseases affecting low- and middle-income countries.
Tenho trabalhado com aspectos imunológicos, clínicos e terapêuticos da leishmaniose tegumentar Americana na área endêmica de Corte de Pedra, localizada no sudeste da Bahia, Brasil, nos últimos 28 anos. Comecei minhas atividades de pesquisa durante minha bolsa em Imunodermatologia no INSERM U209 em Lyon, França, onde tive a oportunidade de estudar a imunopatogênese de doenças bolhosas autoimunes. Depois de voltar ao Brasil, comecei a estudar os aspectos clínicos e imunológicos da hanseníase e da leishmaniose tegumentar, incluindo aspectos específicos da doença precoce, ou formas graves, como a leishmaniose mucosa e disseminada. Durante esse período, nosso grupo do Serviço de Imunologia da Universidade Federal da Bahia investigou vários aspectos da imunidade do hospedeiro na leishmaniose tegumentar, contribuindo para o entendimento de que um dos principais fatores associados ao dano tecidual e à doença é a resposta inflamatória imune exagerada. Como conseqüência, temos proposto novas abordagens terapêuticas, como GM-CSF e pentoxifilina, associadas à quimioterapia para aumentar a taxa de cura do tratamento da leishmanose cutânea e mucosa. Nos últimos anos estive envolvido com a concepção e implementação de ensaios clínicos em diferentes formas de leishmaniose tegumentar Americana, com especial interesse em imunoterapia associada a tratamento específico, e também na avaliação de novas drogas anti-Leishmania.
O impacto da leishmaniose tegumentar na região endêmica de Corte de Pedra está associado a várias consequências sociais, econômicas e psicológicas, constituindo um importante e urgente desafio de saúde pública. Portanto, estou entusiasmado e ansioso para participar do projeto ECLIPSE, cujo objetivo é o de explorar esses aspectos, a fim de garantir um impacto positivo nas comunidades rurais afetadas por essa doença negligenciada. A Universidade Federal da Bahia tem um forte compromisso com a transformação social por meio de atividades e oportunidades educacionais. O estudo ECLIPSE alinha-se bem com a estratégia da nossa Universidade para aumentar a colaboração internacional e promover o desenvolvimento sustentável na saúde pública, com atenção especial às doenças endêmicas que afetam as comunidades de baixa renda.
